2nd Wednesday of every month (till the posts are filled) at 10:00 am (Candidate should reach the venue at 9:00 am). If the 2nd Wednesday is a non-working day, the Document Verification/interview will be held on the next working day.

Application forms can be submitted in 0/0 Dy Civil Surgeon (NHM), Hisar on any working day till 10:00 am on the day of Document Verification/Interview with Demand Draft (DD) @ Rs 500/- perform in case of General Category and Rs. 250/- in other reserved categories in favour of ‘District Health Society NHM Hisar State Salary” payable at Hisar which will be non-refundable. No Cheque or cash will be accepted. The candidate will mention the name, father’s name, category (Gen/SC/BCA/BCB/EWS, etc), and name of the post on the back side of the Bank Demand Draft. The demand draft must be between the date of advt. and date of Document Verification/interview.

DHFWS Hisar Recruitment: Documents Required
- Original documents in support of academic qualifications and work experience shall be required to be produced at the time of Document Verification/interview.
- 1 set of photocopies of all relevant documents along with 2 passport-size photographs on each set shall be required to be submitted.
- Candidates to provide mark sheets for all posts/qualifications (including DNB) are mandatory for the purpose of merit calculations.
- Preference will be given to the candidate residing in the local area (District Hisar) and proof of residence [any three from Passport, Voter Id Card, Ration Card with Address, Caste and Domicile certificate with address and photo issued by State Govt., Parivar Pehchan Patra, Bijali Bill (not older than last three months)] will be required.
- In the case of a married female candidate, after marriage residence proof will be considered for local area weightage.
